The race begins near the Osaka Prefectural Offices and ends within the grounds of Osaka Castle Park. Along the way, participants pass several well-known landmarks, including the Dotonbori area and Kyocera Dome, all while being cheered on by enthusiastic crowds lining the streets. A shorter 7.2 km event, known as the "Naniwa Marathon," also takes place the same day, with options for wheelchair athletes, making the event inclusive and welcoming for various levels of runners.

Recognized by the Japan Association of Athletics Federations, World Athletics, and AIMS, the Osaka Marathon adheres to international standards while offering a uniquely local experience. Entry is typically managed by lottery, with domestic and international runners applying several months in advance. Charity entries are also available, supporting a wide range of causes.
